This new section is intended to make our Troop 109 documents and presentations easily available. Please let us know if other information should be included.

T109 Annual Forms REQUIRED (New!!)
Annual Health and Medical Form has been updated for 2011!! Parts A and B are to be completed at least annually by participants in all Scouting events. Part C is the physical exam that is required for any event that exceeds 72 consecutive hours, for all high-adventure base participants, or when the nature of the activity is strenuous or demanding. See the Boy Scouts of America for more details.
Tips for completing medical form annually: BEST way is to download the form below. You can fill out using pdf file and SAVE for this year and subsequent years. You can even sign the form electronically. Next year, it will be that much easier to update the form and sign again. This form can be used for Philmont and ANY Boy Scout activity.
The previously-used T109 Hold Harmless Form is NO LONGER needed since the consent is now included in the Health and Medical Form.
Pacific Skyline Council Fair Share Campaign -- The Pacific Skyline Council, of which T109 is a member, accepts donations annually to help support scouting programs. Participation is optional but encouraged.
BSA Tour Plan 2011 (New!!)
BSA Tour Plan 2011 -- Required for troop activities. PDF form can be completed and sent electronically for approval.
